Submission Form

 Notes

The aim of the Alliance Resources centre is  to make available in electronic form as many as possible of the documents member associations use in their daily work.

This might include brochures, advice pamphlets,  magazines, reports, etc. - materials for people living with ALS/MND, carers, health professionals,  researchers,  volunteers and staff within your organisation.

If you would like to submit documents for storage and copyright free use by other members of the International Alliance of ALS/MND Associations please follow the instructions below.

1. For each resource you wish to submit, either print out out and complete a copy of Form A or follow this link to fill in on line.

2. Ensure that, for each of the items listed, there are no copyright  issues that would stand in the way of free use by other Alliance members.

3. Compile one hard copy of all items.

4. Print out and complete Form B - Waiver form which will cover all your current and future submissions.

5. Send copies of Forms A, B and electronic versions (if possible) of your documents by email to: info@scotmnd.org

6. Send by Mail in one package:

  •     Paper copies of all materials.
  •     Materials on Disk or CD Rom (Please run a virus check on your disk before sending it to us!)
  •     Copies of Form A for each item submitted
